PCC Application Process and Forms

To apply to the Advanced Studies Program, a completed application (5 pages) must be submitted to your guidance office by the deadline date that your school system establishes. All applications processed by the member school districts are to be submitted to the PCC Office by February 3, 2012.

Students from towns that are not part of the PCC member districts may also apply to the program and will be accepted on a seat-available basis once our in-district students have been accepted and enrolled. Out-of-District student tuition is $TBA.

Our member school’s guidance offices may download the catalog and application materials from our website. Out-of-district parents/students may call the office for information and application materials. (508-279-3404)

Special Notes on Health Forms:

PCC will accept the Well Child Physical/Camp Forms from a Licensed Physician, including immunizations, if all information is present and within the past 24 months. This would be in place of the First PCC Student Health Form. These forms can be mailed, faxed, or emailed to PCC if the physician's HIPPA policy allows.
